Hahaha, in all honesty, you are doing absolutely fine. If I were to learn Japanese, my condition would be much worse than yours. At least you have the grasp of English, meanwhile I know nothing about Japanese.
If you can't tell, I am attempting to write in a simple format, so it may be easier for you to recognize and understand English phrases, rather than to use a translator. I advise you do not use a translator because that basically teaches you nothing. The best thing you can do right now is come to English websites like Newgrounds, talk with some of the people (Like I have seen you do on the BBS), and learn the core grammar. This will bring you success.
As for the doctor, my opinion is to go see one. Knowing the truth is more desirable than to wonder the possibilities of your condition. Your sickness could be minor, or it could be serious. However, both of these options (in my opinion) is better than the fear you experience about the unknown sickness.

I'll admit you're english is rather good, a large diction is very nice to read. As for the doctor thing I doubt the'll do anything bad to you =/ I suggest you have it checked out. No need to worry if you go to a good hospital then there will be no need to fear anything at all. No poison no harrasment nothing, that is unless the doctor wants to lose his/her licence that would be utterly stupid of them though.

Also about the doctor phobia thing, I understand that you are worried. Ask a relative to join you or best friend, going alone is possibly the worst thing you can do. At this time you need emotion soupport by anyone.
